The expansions are the only content that you have to buy with real money, everything else can be had using in-game gold if you own a non-free copy of the game. If you're poor you can find a good guild and they will usually help you cover the costs. Basically the game is split between boxed and DLC content to allow for frequent updates. You should at least buy Season 2 before playing HoT, the rest can come later.
